Claim Check Forwarding Ending Soon

As a reminder, in April 2023, VSP^® changed the mailing address to submit paper claims. Beginning on January 31, 2024, claims sent to the old claim address will no longer be forwarded and will be returned to the sender as undeliverable by the post office. Please use the new address listed below to ensure your claim is received.

Claims will continue to be forwarded until January 31, 2024, and will experience a delay of 3 – 5 business days as they get forwarded. After January 31, 2024, claims sent to the old address will not be forwarded.
